T110 Change event decision sheet
Change event decision sheet

Each change event logged on the Ridgeview Medical Office Building mechanical package, with its estimate, the hours already worked and the decision Brackenhurst Builders has to make, so the concealed duct conflict is authorized or declined before more hours are spent. Prepared by Dominic Ostrowski, Ironvale Mechanical project manager, for the Brackenhurst project manager and Anjali Venkataraman.
Nine change events have been logged since mobilization. Five are authorized, 2 are declined and 2 wait on a Brackenhurst decision. The working rule is plain: no extra work before written authorization.
Sample template: the firm, people, clients and figures are fictional, and the embedded view holds synthetic sample records.
The nine change events
The table lists every event with its decision, its estimate and the hours crews worked before the decision. Eight events show 0 hours. One shows 14.

The work orders behind each event
The view below holds 13 work orders for the Ridgeview Medical Office Building, all for Brackenhurst Builders: WO-3101 to WO-3109 are the nine change events, and WO-3110 to WO-3113 are four inspection items. Read Status and Customer approval together.

Five records show Needs follow-up, and five show Customer approval as Pending: the concealed duct conflict, the thermostat wiring relocation and three inspection items.
Five authorized, two declined
Authorized events are priced, approved inside Ironvale and approved in writing by Brackenhurst. Their work orders show where the work stands.
| Change event | Work order | Estimate | Work order status | Customer approval |
|---|---|---|---|---|
| Rooftop unit curb adapters | WO-3101 | $12,400 | ||
| Hydronic isolation valves | WO-3102 | $9,800 | ||
| Exhaust fan riser relocation | WO-3103 | $7,100 | ||
| Condensate pump addition | WO-3104 | $5,600 | ||
| Floor drain addition | WO-3105 | $3,700 | ||
| Pipe insulation upgrade | WO-3106 | $2,200 | ||
| Access panels addition | WO-3107 | $1,500 |
The five authorized events total $38,600. The two declined events were priced at $2,200 and $1,500, and no crew time went into either. The rooftop unit curb adapters are a separate matter from the rooftop unit curb supply question: the adapters are authorized, and the supply question sits in the scope review with Anjali Venkataraman.
From logged to authorized
Of 9 events logged, 2 were declined and 2 are still waiting, which leaves 5 authorized. The waiting two are the only events that can still change the authorized total.

Decision states
The concealed duct conflict
Concealed duct conflict: priced at $7,450, further work stopped
Crews found the conflict in the third floor ceiling space on Sep 22. Ironvale priced the change at $7,450. Before it was priced, crews logged 14 labor hours on it, because the foreman had been told to keep the floor moving. That broke the working rule of no extra work before written authorization.
What Ironvale did. Dominic Ostrowski priced the change and stopped further work on it. The work order WO-3108 shows Needs follow-up and Urgent. What Brackenhurst decides. Anjali Venkataraman decides authorization at the Thursday meeting, and the decision is recorded by Oct 6. Whether the 14 hours are part of the authorized price is part of that decision, and Ironvale does not assume they will be. Consequence. Until the decision is recorded, the third floor ceiling space stays as it is and no more hours go on this event.
The second waiting event is the thermostat wiring relocation, WO-3109, estimated at $3,200. Its status is New and its owner is Marguerite Toussaint. No work starts on it until Brackenhurst decides.
