T97 Annual relationship review
Annual relationship review

A year of service for Anne and Gerald Abernathy on one page: the reviews held, the requests completed, the decisions made and how long each kind of request took, with the one open question named. Prepared by Thomas Whitaker, lead adviser at Quillfield Wealth Advisors, with Rebecca Stroud, client service director, for the Abernathy household. It covers October 2025 to September 28, 2026, and is the starting point for the annual review on Thursday, October 15, 2026.
Sample template: the firm, people, clients and figures are fictional.
The year in four counts
Two of the four counts are about the October 15 review: how many accounts are reconciled and how many requested items have arrived. The other two are about the year: how fast requests are completed and how many spring decisions are finished.

Requests through the year
Quillfield completed 19 service requests for the household in the 12 months. The most in one month was three, in April, the month of the spring review. The two requests completed in May were the higher monthly distribution and the beneficiary update from that review.

How long each kind of request takes
The median is 3 business days across all 19 requests. Two of the 19 took more than 15 days. Rebecca Stroud keeps the reason for each in the service log and will go through both on October 15 if Anne or Gerald ask. Distributions and transfers take longest because the custodian has to accept the paperwork.

| Request type | Median business days | What it covers |
|---|---|---|
| Distributions and transfers | 6 | Changes to the monthly distribution and moves between accounts |
| Account changes | 4 | Registration, address and account details |
| Documents and forms | 3 | Statement copies and signed forms |
| Beneficiary updates | 2 | Changes to the people named on an account |
| Questions | 2 | Questions the household sends between reviews |
Service in the year
The table lists the larger pieces of service work. Each one has an owner and a status.
| When | Service | Owner | Status |
|---|---|---|---|
| Oct 2025 | Annual review held and notes filed | Thomas Whitaker | |
| Apr 21, 2026 | Spring review held and three decisions recorded | Thomas Whitaker | |
| May 2, 2026 | Monthly distribution raised | Thomas Whitaker | |
| May 20, 2026 | Beneficiaries updated on every account | Rebecca Stroud | |
| Aug 19, 2026 | IRA consolidation transfer request logged | Rebecca Stroud | |
| Sep 10, 2026 | Six review items requested through the secure request | Zaid Bukhari | |
| Sep 14 to 21, 2026 | Family trust account opening started | Rebecca Stroud | |
| Sep 17, 2026 | 401k rollover deposit added to the open items | Linnea Holmqvist |
The IRA consolidation is the one spring decision still open. The receiving custodian rejected the first transfer form because of a signature mismatch. Rebecca sent a corrected form for Anne to sign and logged the delay against the decision.
One question has no recorded answer
The question. In the spring the household asked about funding long-term care. No answer has been recorded since. Consequence. The household has waited since spring for it, and a year of service should not close with an open question. Owner and next step. Thomas Whitaker added the question to the October 15 agenda and noted the delay. A related item is still outstanding: the long-term care policy statement from the insurer, which Zaid Bukhari follows up on Friday, October 2.
A year of service should be visible on one page, including the part we have not finished. The long-term care question is mine to answer on October 15.
