T96 Account opening and transfer checklist
Account opening and transfer checklist

What the custodian requires to open the Abernathy family trust account and to start moving the existing brokerage account into it, in plain terms, with who holds each item. Kept by Rebecca Stroud, client service director at Quillfield Wealth Advisors, for the two trustees, Anne and Gerald Abernathy. Status as of Tuesday, September 29, 2026. Issued Thursday, October 1, 2026.
Sample template: the firm, people, clients and figures are fictional, and the embedded view holds synthetic sample records.
Where the 11 items stand
Eight of the 11 items are complete. The three that remain are the trust certification and one identification document for each trustee. The custodian will not open the account until all 11 are in. Each segment of the ring below is one group of items.
Live example from the Dashboards app, shown with the Abernathy household's demo data. Block dates move forward each week, so they can differ from the sample dates on this page.
What the custodian's words mean
Trust certification. The custodian's form that confirms the trust's name, its date and who the trustees are. Both trustees sign it.
Registration type. How the account is titled at the custodian, so that the account and the trust agreement match.
Transfer. Moving the existing brokerage account into the new trust account. It can start only after the trust account is open.
Signature mismatch. A signature on a form that does not match the one the custodian holds. It is why the first IRA transfer form was rejected.
Decisions and account opening
The view lists the decisions and the account-opening items for six households. Read the Status column: Complete means the work is done, Waiting on household means the next move is the household's, Waiting on third party means a custodian, attorney or plan holds it, and In progress means Quillfield is working on it. It holds 15 records. Seven belong to the Abernathy household: four are Complete (Raise the monthly distribution, Update beneficiaries on all accounts, Trust agreement copy, Account application for the trust) and three are Waiting on household (IRA consolidation transfer form, Trust certification signed by both trustees, Identification documents for both trustees). The two identification documents sit on one record.
Before you share this page with a client
This sample view holds the records of several households: the Yusuf, Dufresne, Iverson, Castellano and Moreno households as well as the Abernathy household. Switch the embed to a view filtered to the household you are sharing with before you send this page to its trustees.
How the transfer runs
The sequence below is the plan for the trust account. Quillfield checks every signature line and every document before anything goes to the custodian, because the first IRA transfer form came back for a signature mismatch.

participant H as Household trustees
participant Q as Quillfield

Q->>H: Send the trust certification and the identification request (Sep 21)
H->>Q: Both trustees sign, and each uploads one identification document
Q->>Q: Check every signature line and every document
Q->>C: Submit the 11 opening items
C->>Q: Confirm the trust account is open (target Oct 20)
Q->>H: Send the transfer instruction for both trustees to sign
H->>Q: Return the signed transfer instruction
Q->>C: Start the transfer of the brokerage account
C->>Q: Confirm the transfer is complete
Q->>H: Report that the transfer is complete
The trust certification still needs both trustee signatures
The custodian will not open the family trust account without a trust certification signed by both trustees. Thomas Whitaker confirmed the requirement with the custodian, and Rebecca Stroud sent the form for both signatures. Owner: Rebecca Stroud. Consequence: until both signatures are in, the account cannot open by October 20, which puts a transfer start before year end at risk. Next step: Anne and Gerald sign the form and return it through the secure request. Rebecca checks each signature line before submitting it.
Approval gates
Some steps need a named person's approval before the next step can start. The gates below show the three that matter for the household today: the higher monthly distribution, the IRA transfer form and the trust certification.
Live example from the Dashboards app, shown with the Abernathy household's demo data.
The IRA transfer form is the gate to compare with the trust certification. The receiving custodian asked for changes, and the corrected form is with Anne to sign. Both gates wait on a signature, and both are owned by Rebecca Stroud.
Target dates
What this checklist covers
This checklist covers what the custodian requires and who holds each item. It does not recommend how to title or fund an account, and it contains no investment, tax or legal advice. Questions about the trust itself go to the household's trust attorney.
Who is on this checklist
How to use this template
Keep one checklist for each account opening or transfer. List every item the custodian requires when the account enters implementation, record who holds each item and mark it complete only when the custodian's requirement is met.
| Variable | Meaning | Required |
|---|---|---|
{{household.name}} | Household the account is for | Required |
{{account.type}} | Type of account being opened or transferred | Required |
{{requirement.rows}} | Each custodian requirement with holder and status | Required |
{{requirement.missing}} | Requirements still missing, with owner and consequence | Required |
{{signature.holders}} | People who must sign each form | Required |
{{date.target_open}} | Target date for the account to open | Required |
{{date.target_transfer}} | Target date for the transfer to start | Optional |
If a required value is missing, show Needs review in its place; never guess a requirement or a date. If the custodian's requirement list cannot be read, show Unavailable and do not mark any item complete.
