T95 Client decision follow-through record
Client decision follow-through record

What happened after Anne and Gerald Abernathy made three decisions at the April 21, 2026 review: the service work each decision created, who owns it, where it stands and why one of them is late. Kept by Rebecca Stroud, client service director at Quillfield Wealth Advisors, for the Abernathy household and for Thomas Whitaker, lead adviser. Status as of Tuesday, September 29, 2026. Issued Thursday, October 1, 2026.
Sample template: the firm, people, clients and figures are fictional.
The three decisions and their service work
A decision is not finished when the meeting ends. Each tab below shows one decision, the service work it created, the owner of each piece and where each piece stands.
The decision. At the April 21 review the household decided to raise the monthly distribution. The result. The new monthly distribution started in May and is reported as complete.

| New monthly distribution started | Custodian | May | |
| Reported as complete at the review | Thomas Whitaker | Oct 15 |
The decision. At the April 21 review the household decided to update the beneficiaries on all accounts. The result. The update was completed on every account in May, the last on May 20. The household confirmed its beneficiaries again through the secure request for this review.
| Service work | Owner | Date | Status |

The decision. At the April 21 review the household decided to consolidate Gerald's two IRAs. The result so far. The two IRAs are still separate. The receiving custodian rejected the first transfer form because of a signature mismatch, and the corrected form is with Anne to sign.
| Service work | Owner | Date | Status |
|---|---|---|---|
| Decision recorded in the service record | Thomas Whitaker | Apr 21 | |
| Transfer request logged for Gerald's two IRAs | Rebecca Stroud | Aug 19 | |
| First transfer form rejected: signature mismatch | Receiving custodian | Rejected | |
| Corrected form sent to Anne to sign | Rebecca Stroud | Sent | |
| Anne signs the corrected form | Anne Abernathy | Before Oct 15 | |
| Corrected form submitted to the receiving custodian | Rebecca Stroud | Before Oct 15 | |
| Transfer accepted by the receiving custodian | Receiving custodian | After submission |
Why the IRA consolidation is late
Reason. The receiving custodian rejected the first transfer form because of a signature mismatch. Rebecca Stroud sent a corrected form for Anne to sign and logged the delay against the decision. Consequence. The two IRAs stay separate until the custodian accepts a corrected form, so the review packet reports this decision as open on October 15, not complete. The consolidation has waited 41 days. Owner and next step. Rebecca Stroud: Anne signs the corrected form, then Rebecca submits it before the October 15 review.
How a decision moves
Every decision follows the same path. It waits in one of two places, with the household or with a third party such as a custodian, and returns to In progress when the item arrives. A decision that waits more than 14 days gets a written reason and a named owner on this record.

A decision is finished when the work is done. If it is late, the reason and the owner belong on the record next to it, where the household can read them.
