T93 Household review packet
Household review packet

The packet Thomas Whitaker opens for the Abernathy annual review on Thursday, October 15, 2026: the accounts as reconciled, the items still open, the commitments from the April 21 review and the questions for the meeting. Anything not yet confirmed is named as open and never estimated. Prepared by Rebecca Stroud and Linnea Holmqvist at Quillfield Wealth Advisors for Anne and Gerald Abernathy, from records checked on Monday, September 28, 2026. It goes to the household once Thomas has reviewed it. Issued Thursday, October 1, 2026.
Agenda for October 15. The step marks show how far each section of the packet is prepared today.
Sample template: the firm, people, clients and figures are fictional.
Accounts in the packet
The household holds seven accounts. Five are reconciled to the custodian's records. Two are open, and each one is named with its reason. An account that is not reconciled is listed as open and carries no estimate.
| Account | Type | What the packet uses | Status |
|---|---|---|---|
| Joint brokerage account | Taxable brokerage | Reconciled to the custodian record on Sep 25 | |
| Anne Abernathy individual brokerage account | Taxable brokerage | Reconciled to the custodian record on Sep 25 | |
| Gerald Abernathy traditional IRA | IRA | Reconciled on Sep 25; the receiving account for the IRA consolidation | |
| Gerald Abernathy second IRA | IRA | Reconciled on Sep 25; the two IRAs are still separate | |
| Gerald Abernathy individual brokerage account | Taxable brokerage | Reconciled to the custodian record on Sep 25 | |
| Rollover account from Gerald's former employer 401k | Employer plan rollover | Deposit confirmation from the custodian | |
| Variable annuity | Annuity | September statement from the annuity provider |
Read the chart below from left to right. Seven accounts are held, one is taken out because the rollover is not yet reflected, one more because the annuity statement is late, and five remain reconciled for the packet.

Last year's commitments
Three decisions came out of the April 21 review. Two are complete. One is waiting on the household. The notes from last October's annual review and from April 21 are filed behind this section for Thomas to open during the meeting.
The consolidation is the only commitment that carries over. The receiving custodian rejected the first transfer form because of a signature mismatch. Rebecca sent a corrected form for Anne to sign and logged the delay against the decision. The packet reports the consolidation as open on October 15, not as complete.
Open items, named and not estimated
The 401k rollover is listed as open, not estimated
Rollover account. The rollover check from Gerald's former employer was mailed to the household instead of the custodian, so the new account shows no balance. Rebecca Stroud listed the rollover as an open item in this packet instead of showing an estimated balance. Owner: Linnea Holmqvist, who confirms the deposit with the custodian before October 15. If the deposit has not posted by then, the account stays listed as open and Thomas explains where it stands at the meeting.
Variable annuity. The September statement from the annuity provider is late, so the annuity is not yet reconciled for the packet. Owner: Linnea Holmqvist, who requests the statement from the annuity provider.
I would rather show Anne and Gerald an open item than a number nobody has confirmed. The packet only states what the custodian has put in writing.
Seven items are open before the review. Days waiting are counted on September 29.
| Open item | Owner | Waiting | Next step |
|---|---|---|---|
| IRA consolidation transfer form | Rebecca Stroud | 41 days | Submit the corrected form before the Oct 15 review |
| 2025 tax return | Zaid Bukhari | 19 days | Receive the return by Oct 9 |
| Long-term care policy statement | Zaid Bukhari | 19 days | Follow up with the insurer on Oct 2 |
| 401k rollover deposit | Linnea Holmqvist | 12 days | Confirm the deposit with the custodian before the review |
| Trust certification signed by both trustees | Rebecca Stroud | 8 days | Open the trust account by Oct 20 |
| Identification documents for both trustees | Rebecca Stroud | 8 days | Upload both documents through the secure request |
| Variable annuity statement | Linnea Holmqvist | 6 days | Request the statement from the annuity provider |
The count below is the one the household sees on the first page of the packet.

What this packet does and does not do
The packet reports the state of accounts, documents and service work. It shows no estimate for an account that is not reconciled. It contains no investment, tax or legal advice: analysis and recommendations are Thomas Whitaker's, given at the meeting.
