T91 Completion and certificate review
Completion and certificate review

Each of the 24 pilot learners set against the completion rule, with the evidence each decision rests on and the gap named wherever there is one: a certificate is issued when the evidence is complete, not when the attendance looks good enough. Prepared by Joaquin Ibarra, program operations director at Saltbridge Academy, for Olivia Holloway, head of learning and development at Kittridge Hotels Group. Prepared Thursday, October 1, 2026, from the learner records of Monday, September 28.
Sample template: the firm, people, clients and figures are fictional.
Where the 24 learners stand
Each square is one learner in the pilot cohort. Nineteen meet the rule and have their certificate. Three meet attendance and assignments and are missing only the manager sign-off. Two fall short on attendance. That is a completion rate of 79 percent today, 19 of 24. If all three sign-offs arrive, 22 of 24 would meet the rule.

Evidence for each learner group
Every decision cites what it rests on. A group is decided together only when its evidence is the same.
| Group | Learners | What the record shows | The gap | Decision | Owner and date |
|---|---|---|---|---|---|
| Meet the rule | 19 | Attendance at or above 80 percent, four assignments submitted and a manager sign-off on file | None | Certificates issued | Haruka Okada, done |
| Waiting for a manager sign-off | 3 | Attendance and four assignments meet the rule | Manager sign-off | Certificates held until the sign-off arrives | Joaquin Ibarra asked the three hotel managers; sign-off needed by Oct 9 |
| Short on attendance | 2 | Attendance below the 80 percent minimum | Attendance | Open: catch-up extension or no certificate | Haruka Okada with Olivia Holloway, decision by Oct 9 |
From enrollment to certificates
Start with the 24 learners enrolled in the pilot. Take away the 2 who are short on attendance and the 3 waiting for a manager sign-off. What is left is the 19 certificates issued. The two steps down are different problems: one is evidence that is missing from a hotel, the other is a decision still to be made.

Three certificates are waiting for a manager sign-off
Three learners have met attendance and completed all four assignments. The rule also needs a manager sign-off, and the three hotel managers have not given it yet. No certificate is issued until they do. Joaquin Ibarra asked the three managers for sign-off by October 9. On the sponsor action list this request had been with the hotels for 11 days on September 29.
Consequence: until the sign-offs arrive, three learners who did the work have no certificate. If a sign-off has not arrived by October 9, Joaquin raises it with Olivia Holloway that day, and the certificate stays unissued until the sign-off is on the record.
The two attendance cases
Two learners fall short on attendance. The choice is between a catch-up extension and no certificate. Haruka Okada and Olivia Holloway decide by October 9. The tabs set out what each option needs, so the decision rests on evidence.
What it means. The learner makes up the missed sessions by a set date, through make-up sessions or recordings with a check by the facilitator, and the attendance record is updated. If attendance reaches 80 percent, the assignments and the sign-off are checked and the certificate follows.
Evidence it rests on. The sessions made up, with dates and the facilitator's check, recorded against the attendance record. The completion rule itself does not change.
Watch for. A recording is not live attendance. Olivia Holloway and Haruka Okada agree before the extension starts that make-up sessions count, and the same terms are offered to both learners.
Date. Terms and end date agreed by Oct 9.
What it means. The learner does not receive a certificate for the pilot cohort. The record stays as it stands. Haruka Okada tells the learner and the hotel manager which requirement was not met and what the evidence shows.
Evidence it rests on. The attendance record against the 80 percent minimum, with the sessions missed and their dates.
Watch for. This fits a gap that make-up sessions could not close fairly. Say so in the record, and name the route to challenge the decision (see the box at the end of the review).
Date. Decision recorded by Oct 9, with Olivia Holloway's agreement.
Settle four questions for each of the two learners, with the evidence in front of you.
- How far below 80 percent is the attendance?
- Why were the sessions missed?
- Does the rule allow make-up sessions to count?
- Is the same option fair for both learners?
Haruka Okada decides completion as the qualified instructor, and Olivia Holloway agrees the option for Kittridge. Record the decision, the date and the reason, then tell the learner and the hotel manager.
Evidence by requirement
Each bar counts learners with complete evidence for that requirement, out of the 24 in the pilot cohort. The two attendance cases are counted as open on assignments and sign-off until the attendance decision is made, because their evidence is reviewed together.
If a decision is challenged
The learner, the learner's hotel manager or Olivia Holloway can query any completion decision. Haruka Okada re-reads the evidence the decision rests on, and Joaquin Ibarra records the query, the outcome and the reason on the cohort record. A decision stands until that record says otherwise.
Decisions still to make
How to use this template
Run one review per cohort when it finishes, against the completion rule the client approved. Group learners by what the evidence shows, name each gap, and issue a certificate only when attendance, assignments and sign-off are all on the record.
| Variable | Meaning | Required |
|---|---|---|
{{cohort.name}} | Cohort under review, with finish date and learner count | Required |
{{rule.requirements}} | Attendance, assignment and sign-off requirements of the completion rule | Required |
{{rule.approved_date}} | Date the client approved the completion rule | Required |
{{learner.group_counts}} | Learners who meet the rule, wait for a sign-off or fall short on attendance | Required |
{{evidence.rows}} | For each group, what the record shows, the gap, the decision and the owner | Required |
{{signoff.waiting}} | Sign-offs still owed, with who owes them and the date | Required |
{{case.options}} | Options for learners who fall short, with the evidence each needs | Required |
{{decision.date}} | Date by which open cases are decided | Optional |
If a required value is missing, show Needs review in its place and hold the review; never fill it with a guess. If the learner records cannot be read, show Unavailable instead of the counts and issue no certificate.
