T128 Accessorial charge evidence pack

Accessorial charge evidence pack

A freight brokerage account coordinator assembling evidence for a detention charge

Status as of Sep 28, 2026 4 events disputed, due Oct 9 5 events lacked carrier arrival times Client-visible

The evidence behind every detention, layover and reconsignment charge on Hollowell Paper Products loads in September 2026, set out before the charge is billed, so each event is accepted or disputed on the record. Prepared by the Quartzline Freight account coordinator for Bettina Kowalczyk, Transportation manager at Hollowell, and Hollowell accounts payable. Marcus Ellery, Key account manager, decides whether each charge is billed, absorbed or disputed. Priyanka Raghunathan, Operations manager, validates the facts. Issued Thursday, October 1, 2026, from records checked on Monday, September 28, 2026.

Sample template: the firm, people, clients and figures are fictional, and the embedded view holds synthetic sample records.

September by decision

Twenty-seven accessorial events were raised on Hollowell loads in September, worth $9,840 in all: 19 detention, 5 layover and 3 reconsignment. The bars show where each one stands. Sixteen were billed and accepted, 4 are disputed, 3 were absorbed by Quartzline for $1,120, and 4 await a decision.

Evidence before billing

A charge is only as good as the papers behind it. For each event the pack holds the appointment time, the carrier timestamps, the agreement terms and the messages exchanged, and it names any gap before the charge is billed. Evidence was complete before billing on 20 of the 27 events. Seven had a gap. Five of those lacked a carrier arrival timestamp, because the carrier reported its arrival by phone and nobody recorded the time.

A charge with a gap in it is a conversation. A charge with the carrier time, the gate log and the agreement clause attached is usually a signature.

The four disputed events

The view lists the four disputed September events as records, awaiting the close with Bettina on Oct 9, with the owner, the amount, the evidence status and the date the decision is due. All four are owned by Marcus Ellery and all four are due on Oct 9. Read the Evidence column first: Complete means every item for that event type is attached, and Gap means one item is still missing and named in the record.

Before you share this page with a client

This view is filtered to Hollowell Paper Products. The database also holds records for Bramblewood Beverage and Coyne Building Products. Keep the embed on a view filtered to the client the page is shared with, and never point it at an unfiltered view.

RecordLoadEventPlantAmountEvidenceWhy it is disputed
Detention at the north plant dockQF-31872DetentionNorth plant$260CompleteBettina disputes the dock appointment time. Carrier timestamps show 6 hours on site, so 4 billable hours at $65 after 2 free hours.
Detention on the afternoon pickupQF-31955DetentionNorth plant$390CompleteBettina disputes the plant ready time. Carrier timestamps and the plant gate log agree on 8 hours on site, so 6 billable hours at $65.
Layover after a missed appointmentQF-32018LayoverOther plant$350GapOne carrier timestamp is missing. The plant gate log is the proposed evidence, and Bettina agreed gate logs count under the agreement.
Reconsignment of a delayed loadQF-32071ReconsignmentNorth plant$480CompleteThe delivery address changed after pickup. Bettina disputes whether the change came before pickup. The change email and the carrier invoice are attached.

The four records add up to $1,480 (a sum of the four amounts in the table). They are the open slice of September. The other 23 events are billed and accepted, absorbed, or in the separate group of four that still await a decision.

Disputed events and the absorbed $1,120

Four events are disputed and stay open until Oct 9. Three events were absorbed by Quartzline for $1,120 and will not be billed to Hollowell. Absorbing a charge is a commercial decision by Marcus Ellery, recorded with the reason. It does not change the agreement terms for later loads.

Evidence items for each event type

Every event carries the items for its type. Open the tab for the event type, and tick each item only when the document is attached and readable.

Detention

✓Carrier arrival timestamp, recorded at the time it was reported
✓Carrier departure timestamp

✓Agreement clause: two free hours, then $65 per hour with carrier timestamps
✓Messages with the plant and the carrier about the delay
Layover

Carrier arrival timestamp, or the plant gate log when the carrier time is missing
✓Rebooked appointment confirmation
✓Agreement clause on layover
✓Carrier invoice for the layover day
Reconsignment

✓Change request email with its sent time
✓Pickup timestamp, to show whether the change came before or after pickup
✓Carrier invoice for the added miles or the added stop

The open box on the layover tab is the one gap in the four records: the carrier reported its arrival by phone on QF-32018, and the plant gate log stands in for the missing time.

From carrier arrival to billing

The path is the same for every event, and billing waits for the shipper's response. The steps below show it for the four events still awaiting a decision; the 16 accepted events have already been through it.

Where the pack stands

Evidence was complete before billing on 20 of 27 events. The five gaps in carrier arrival timestamps are the reason the count is not higher.

How to use this template

Build one pack for each billing cycle, before any accessorial charge is invoiced. List every event with its evidence status, and name each gap with its owner instead of filling it in.

VariableMeaningRequired
{{account.name}}Shipper the pack is forRequired
{{period.label}}Billing period the events fall inRequired
{{event.rows}}Each event with load, type, amount, evidence status and decisionRequired
{{event.evidence_gap}}The missing item for each event with a gap, with its ownerRequired
{{agreement.terms}}Free time, hourly rate and accepted evidence under the agreementRequired
{{decision.owner}}Person who decides bill, absorb or disputeRequired
{{decision.due_date}}Date the shipper response is dueOptional

If a required value is missing, show Needs review in its place and hold the pack; never guess a time, an amount or a clause. If the event record cannot be read, show Unavailable for the event rows.
